Clock-in and clock-out segments against a work order, per worker — or, lacking one, per account, named from created_by instead.

duration_minutes is generated from the two timestamps. An open segment has ended_at null. One person may hold several open at once, on different jobs, and the segments may overlap in time; a job has at most one open segment whoever holds it (ux_work_order_time_open_per_job).
